# Do statins actually cause muscle symptoms in patients who report them?

**Summary:** In randomised n-of-1 trials, muscle symptom scores were no different on statin than on placebo, pointing to a nocebo effect.

StatinWISE conducted a series of randomised, placebo-controlled n-of-1 trials in 200 patients who had considered stopping or had stopped statins because of muscle symptoms. Each participant alternated between atorvastatin 20 mg and placebo over six two-month periods, rating symptoms throughout [1].

There was no difference in mean muscle symptom scores between statin and placebo periods, and no difference in the proportion of participants withdrawing for muscle symptoms [1]. The finding supports attributing most such symptoms in this population to the nocebo effect rather than to a pharmacological action of the drug.

Practically, two-thirds of participants who completed the trial intended to restart statin therapy afterwards [1]. Individual re-challenge under structured observation is therefore a reasonable option before abandoning statin treatment. This does not apply to genuine statin-associated myopathy or rhabdomyolysis, which are rare, objectively identifiable, and require discontinuation.

## Limitations

- The trial recruited patients with prior symptom attribution to statins, so results describe that population rather than statin-naive patients.
- It tested atorvastatin 20 mg only, and cannot exclude dose-dependent or drug-specific effects at higher intensities; rare true myopathy remains a distinct clinical entity.
